Hospital Occupational Therapist – community focus

Experienced Community Occupational Therapist required for this regional hospital, just two hours from Melbourne. Ideally you will be an experienced clinician who is able to confidently carry a caseload and offer support to the junior OTs on the team. There is a 6 month contract or permanent role available. Ideally to start on October/early November.

Accommodation available for the six month contract.

 
The caseload involves:
  • Working with the frail aged and people with a disability to maximise independence at home.
  • Working with clients, carers and families to provide practical solutions to promote independence and safety at home
  • Developing goals and care plans to assist clients to remain at home and active in their local community
  • Collaborating with other clinicians in the RAHT team to offer holistic care to clients referred
  • Working with a diverse range of clients and health conditions.
  • Working across funding streams such as HACC PYP, CHSP & possibly the NDIS (minimal, if any)
  • Opportunity to work with Allied Health Assistants to undertake client care programs and equipment trials.
  • Primarily home based work with some opportunity for clinic appts.
 
 
Ideally you will have experience across:
  • Home modifications, knowledge of Australian Building Standards
  • Assistive technology/equipment prescription
  • Scooter assessments, Wheelchair prescription
  • Familiar with funding schemes such as Geat2GO and SWEP
  • Community
