- This is a federally funded program which provides restorative, goal-driven care to older adults who are leaving hospital to assist them to return to their home. The program is a maximum of 12 weeks.
- The team is multidisciplinary with AIN, AHA, OT, PT, nursing on the ground and telehealth available for Dietetics and Speech Pathology
- A car is provided for all home visits. A driver's license is a must. Patients can be up to 45 min one way drive from the home base.
- The team employs 1.5 FTE Occupational Therapy, 1 FTE Physiotherapy and around 4 FTE Allied Health assistants
- The OT role sees a client on average once a fortnight and utilises 2-3 x a week AHA visits to carry out clinician-prescribed programs
- Common Conditions include post orthopaedic surgery, frequent falls, functional decline/ deconditioning post lengthy hospital stays, stroke.
- Knowledge of the acute hospital system is preferred, previous experience in community settings including home modification and ADL equipment prescription is strongly preferred. OTs also provide the upper limb rehabilitation for strokes and post upper limb surgery such a shoulder replacement. (Hospital has a hand clinic, so there is no need for knowledge of post tendon repairs or complex splinting)
- The TCP program often discharges to Aged Care Supports. This is an area of great change at present, but a general understanding of the My Aged Care system is also beneficial
Locum rates upto $72 per hour + super.
Upto $500 reimbursed towards travel to the locum.
