Experienced Social Worker required for this large hospital in North Queensland. You will join the long stay team of Social Workers and Occupational Therapists.
The role will be coordinating residential aged care activation and managing an interim facility caseload, subacute / maintenance patients that have left hospital and are temporarily staying in QLD Health funded aged care beds while awaiting permanent placement to RACH. The role involves case management, advice and the prevention of hospital admissions.
Experience can be variable; the ideal candidate will have knowledge of the aged care system and some exposure to working with older people, disability or the NDIS.

- Accommodation provided + one way travel reimbursed
- If you trained overseas, you must have been assessed as eligible for AASW membership
- You must have relevant experience
