Experienced Hospital Social Worker required for this Cairns based service. The role is start ASAP until 26th June. Full time, Monday – Friday 38 hrs per week (7.30-15.36).

- Hospital in the home service. Caseload is predominantly geriatric, navigating services, MAC referrals, patient centred discharge planning, respite and permanent placement applications etc. There is also a component of accommodation, food, service referrals and counselling for younger HITH patients.
- 11 Allied health staff on the team including AHAs, Physiotherapists, Occupational Therapist, Speech Pathologist, Dietitian. The team is super friendly with regular social catch ups.
- HP4 level role, there is a HP3 SW who works on the team (.5 FTE).
- Locum rates of $80-$86 per hour + super + living away from home allowance
- Travel reimbursed upto $1000
- No accommodation available, however there is an allowance of upto $170 per night for accommodation. You will need to organise this yourself.
- Driving license is required as assessments occur at patient's home. Staff use our fleet vehicles to complete home visits within a 50km radius of our community centre (Palm cove to Gordonvale).
