2 hours from Melbourne - Hospital & Community based Occupational Therapists required

 
Permanent and fixed term contracts available.
 
Grade 1 Community OT - permanent or fixed term contract (6 months ++)
 
The Health Ambulatory Aged Care Program (AACP) consists of 3 programs: Transition care program (TCP), Restorative Care Program (REST) and GEM in The Home (GITH).
 
 
Grade 2 Hospital OT - permanent or fixed term contract (6 months ++)
 
Provide services to in-patients, across the acute or sub-acute wards. You will provide specialist assessment, treatment and discharge planning to admitted patients in the Acute, Rehabilitation, Geriatric Evaluation & Management, Palliative Care and Sub-acute in the home 
 
 
Grade 2 Community OT - working in home and clinic settings - permanent or fixed term contract (6 months ++)

  • Work with clients, carers and families to provide practical solutions to promote independence and safety at home
  • Develop goals and care plans to assist clients remain at home and active in their local community
  • Complete equipment prescriptions, home modifications, advise on pressure care management/seating
  • Develop in home and community based therapy programs
  • Work with a diverse range of clients and health conditions
  • Participate in student training programs
  • Participate in service development and quality improvement initiatives
 
Grade 3 Community OT - permanent or fixed term contract (6 months ++)
  • The RAHT assists older people, people with a disability and people with chronic health conditions to remain living at home. Our aim is to help promote patient quality of life, independence and participation in activities. The team also supports carers.
  • In this role you will provide clinical expertise to complex cases, support staff supervision and be responsible for the planning, review and implementation of quality initiatives to enhance the delivery of high-quality Occupational Therapy services within the RAHT service model. The role is a chance to grow your clinical leadership skills and gain exposure in management for career advancement opportunities.
  • You will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home based settings, making a true difference to our client’s independence and well-being. There is no nursing home work.
  • You will report to the RAHT Manager whilst providing high quality social work services consistent with the Occupational Therapy Australia code of ethics and practice standards to clients within the local community.

Relocation Support (for permanent)
  • Intrastate (over 100kms) - up to $2,500
  • Interstate - up to $5,000
  • Overseas - up to $8,000
      
Accommodation Support: 
  • Up to $350 per week up to 10 weeks; or
  • 10 weeks interim Accommodation (if available)
